Here’s How to Pay My Bill Online
You may conveniently pay your bill using this on-line form. An interactive link to the form will be on the bottom of your invoice. Fill the form out in its entirety each time you pay, be certain to include all the required fields. Your information will not be saved on Pay My Bill. It is not recommended that you save your information on your computer or browser and, if you do, log out of any website where you fill this information out. As long as the window is open, your information is at risk. For your safety, I will no longer be keeping card numbers on file.
Payment is due at the time of service by check, cash, Discover, Visa, MasterCard, American Express, HSA/FSA cards, or Zelle®. There is a 5% surcharge on all credit card payments. Payments for all phone consultations and missed appointments are due the date of the appointment. A $40 late fee will apply for all product invoices 30 days past due. A $40 fee will be charged for any unpaid check returned to the office. Payments may be mailed to Myra Nissen, CCH, RSHom(NA), PO Box 162, Benicia, CA 94510.
